Bluerock Care is excited to announce that we have been selected by Medicare to participate in an initiative (‘ACO REACH’) where we can work together with other providers and Medicare to deliver better care.
We have joined an ACO (Accountable Care Organization): Midwest DCE. This will help us provide you with better care coordination, better preventive care, better service and better outcomes. It will also allow us to provide you with enhanced benefits and services, at no cost to you.

Bluerock Care patients with original Medicare (also known as “straight Medicare”) Part B can receive free new benefits*:
*Benefits are available to Bluerock Care patients enrolled in Medicare’s ACO REACH initiative. Conditions, clinical criteria, and need-based criteria apply.
**For patients who do not have access to existing, available transportation benefits, or private transportation.
***This includes remaining copays after secondary insurance coverage.

Please sign this form to tell Medicare that we are your Primary Care Provider.
Instead of returning this form, you can also log into MyMedicare.gov and select Bluerock Care as Your Main Doctor or other health care professional. Click here for instructions for navigating MyMedicare.gov. If you make a selection on this form and make a different selection through MyMedicare.gov, Medicare will prioritize the selection you make through MyMedicare.gov.
ACO REACH will NOT change your Medicare benefits, & you can visit any doctor, other health care professional, or hospital.
You remain eligible to receive the same Medicare benefits and you still have the right to use any doctor, other health care professional, or hospital that accepts Medicare, at any time. The choice to participate or not will also not change our commitment to your care.
Participating in this program is your choice AND you can change your mind later.
